Disability Registration Agreement
This form allows you to acknowledge certain important aspects of the accommodation process as well as to provide information about your disability to the University in a confidential manner. This form should be submitted to your campus Vice-President. This form must be completed as part of the application for accommodation under the ADA. 

Confidentiality Statement
This form outlines the processes by which we keep your information secure and confidential. This form should be submitted to your campus Vice-President. This form must be completed as part of the application for accommodation under the ADA.

Request for Adjustments/Accommodations Form 
This form allows you to provide essential information to the University regarding the nature of your disability as well as describe the accommodations or adjustments that you feel would most effectively serve you. This form should be submitted to your campus Vice-President. This form must be completed as part of the application for accommodation under ADA.

Approved Course Adjustments/Accommodations
This form will be completed by University administration each term and will be signed by your faculty member, you, your Dean, and your Campus Vice-President. You do not need to complete this form as a part of your application to receive accommodation under the ADA
